Usually they are not. They play the notes as written in their printed parts. Starting in the 20th century some composers specifically ask for improvisation within certain set parameters, such as a specified number of measures, or a list of short motives the performer can play in whatever order s/he wants. Such moments of improvisation are always designated in the printed parts. Complete freedom to improvise is very rare.
