Slayer is a really obvious answer here, but bands like The Berzerker, or Napalm Death. Napalm Death actually wrote a song that won the world record for shortest recorded song ever recorded. I believe it was 0.316 seconds long. These bands are both insane.

REAL ANSWER: The original Incubus, (now known as Opprobrium), has the fastest tempo and thrash beats I have ever heard, you can hear it on their album entitled Serpent Temptation. The fastest blast beats I have ever heard come from a band called Sarcófago, they have a couple of songs on the album I.N.R.I. one in particular, the song "Nightmare", that are so fast at times that they sound like drum rolls. There are a number of modern day technical death metal bands that play blast beats that fast but its difficult for me to tell if these guys or the newer stuff is faster. The fastest guitar riffs I have ever heard come from a band called Attomica, the second guitar riff on the song "Samurai" is by far the fastest guitar riff you will ever hear. Most modern extreme metal does not have an emphasis on speed and most bands from today are more interested in heaviness. Also most bands that are extremely fast are underground and difficult to find due to record labels being hesitant to sign them. I also agree with the second answer, Slayer was the fastest of "The Big Four" but they are hardly fast in comparison to a lot of the death metal bands from the late 80's and early 90's, especially the early stuff from Sepultura and Deicide.
As of Yngwie Malmsteen being the fastest shredder in the world, I like his neo-classical influence, but the faster Guitarists such as Trey Azagthoth in his song Summoning Redemption, Mick Barr in his song P art 1, and Michael Angelo Batio win his song Full Force are few and many above Yngwie.
With countless new bands appearing on to the scenes at break-neck speed, who knows for sure who is the fastest?! As I'm typing this message, some German teenage metal-heads in Munich could be distributing demos of their latest work, which for all we know, could be the fastest ever. However, the general consensus is that the fastest thrash album ever released to date, is the 1986 masterpiece that is 'REIGN IN BLOOD' by none other than SLAYER. It's only about 28 mins long, but then again, what do you expect when a guitar solo that is normally 2 mins long, is played lightning quick and brutally to be reduced to a mere 15-20 secs! The whole album is too fast to savor by relaxing on the beach and sipping on a coconut. It will hit you in your face like a concrete slab. Hence, by virtue of creating the fastest thrash album, SLAYER, I would say, is the fastest thrash metal band (in my opinion).

This is a very hard question to answer. There is a sub-genre of Metal called "Speed Metal" and some are really fast - it is hard to decide which is the fastest.
For example, there is a YouTube video of ten speed metal bands which you can watch for get an feeling of how fast Speed Metal Bands are. Search for speed metal on youtube.
If you are referring to fast as in Fast tempos, speedy drum beats, rhythms riffs and solos, their is no real definite answer but i can give you a list of those considered the fastest.

How do you define "top"? Highest grossing? Metallica and Iron Maiden. Best? This is subject to debate. IMO, a band which completely sells out (guess which band I refer to here) is far from being the "best" in Metal music... especially when that band hasn't put out a worthwhile album in the last 22 years, rode on the backs of bands in the NWoBHM to get famous in the first place (such as pinching almost every riff from their debut album from Blitzkrieg, Savage, and Angel Witch), then tries to regurgitate their glory days by cranking out trend-following album after trend-following album, only to turn around and claim them to be a "return to their roots".
If you're looking for a band who never made the sorts of money the "Big Four" made, yet stayed true to their roots and consistent over the course of decades, and is representative of what true Metal is, then Manilla Road is your band.
As the previous respondent posted. What is meant by the "Top" heavy metal band? There are a lot of HM bands who have sold shed loads of records and there are bands that have been more influencial but have had less sales success. Iron Maiden have had the luxury of massive record sales and have been massively influencial in that genre and Motorhead (although now do not have the record sales they did about 30 years ago) were a major influence on the rock and metal scene infusing the aggression of punk with the power of rock. Thin Lizzy (although not really a heavy metal band) had a massive influence on the genre with their "Two lead guitarists and harmonies approach" which has been copied by many metal bands ever since. Judas Priest with their leather, studs, high pitched vocals and aggressive riffs had a major influence on the music and were well before their time.
All in all it's hard to decide who would rank as the "Top Metal Band" but if you look for a band that has had good record sales, been a massive influence and is liked by probably most metal fans then I think you have to look no further than Black Sabbath. After all, if they had not existed then heavy metal may not have existed as well.
