No one really "designed" heavy metal. It all came about with certain bands/artists writing certain songs and using certain styles. A music type has to develope over time.
However, Alice Cooper did created his own genre of music called "shock rock" because of all the songs he created. That is an acception to the rule. However, Alice Cooper did influence many bands that are now categorized as heavy metal today (such as KISS, Marilynn Manson, etc).

Although it's hard to pin point an EXACT year, many will say that Metal was born in 1968-1970, with the release of Led Zeppelin 1-3, Black Sabbath's self-titled and Paranoid album, Deep Purple's self titled and Deep Purple in Rock, Alice Cooper's Pretties for you and Easy Action, and Iron Butterfly's Heavy, Inna gadda da vida, Ball and Live albums. Not long after, Heavy Metal got it's name from the Steppenwolf song "Born to be wild" (Though few see them as a heavy metal band). Again, few can pin point the exact year when heavy metal was formed, or when it got the name "Heavy Metal", but this is the opinion I have, which many people generally agree with

Tough one. Overall, I'd say Ozzy. Ozzy was part of the band that invented metal, after all, and if you take his discography as a whole there's little that isn't classic metal. Alice has recorded things that are way heavier than Ozzy's stuff, but some periods in his career have been exclusively rock.
