I've been researching his death and the things I found point to suicide.
For starters, I'd just like to say that you don't just take 4 different types of pain killers/medications and alcohol together and expect to live:
"The results of the autopsy performed on December 30, 2009, however, were inconclusive.[1] It was not until June 9, 2010 that the cause of death was revealed to have been an acute polydrug intoxication due to combined effects of oxycodone (OxyContin), oxymorphone (a metabolite of oxycodone), diazepam (Valium), nordiazepam (a metabolization of diazepam) and alcohol."
I believe, however, that this combination of drugs and alcohol was more than coincidental. The song fiction does appear to be a goodbye note, but I think that the rev had been contemplating suicide long before the making of nightmare.
