An A chord can be played several different ways on the guitar, but the most common A chord is the open A. To play this chord, simply bar your first finger across the second fret of the second, third, and fourth strings. When strumming this chord, be sure not to strum the sixth string.
I assume you are referring to a C/B chord; this is used as a "passing" chord where you play the top as a normal C chord but the B is you bass (and lowest) note -- use the A string 2nd fret for the B and don't play the lowest E string.
Open High E (1st string), first finger on 1st fret 2nd string (C), open 3rd string, third finger on 2nd fret 4th string (E), second finger on 2nd fret 5th string (B). It sounds weird by itself, but play a normal C --> C/B --> Am and you can see how it's normally used.
In C/G, C means the main chord, and G means the bass note that you play alongside with C

On a keyboard, an F chord triad is f - a - c. On the guitar, you take an E chord and bar it up one fret. (Incidentally, it's f - a - c on the guitar too.)
