It depends on which version of 'The Phantom of the Opera' you are refering to. In Andrew Lloyd Webber's musical, the Phantom sings about how he was born with a skin infection...which caused a "mother's fear and loathing". So it is most recently said to have been a skin infection, except that his musical version was slightly different from the original novel by Leroux. He was born with the deformity. It is clearly implied in the original novel as well as in the stage musical and the 2004 film based on that novel. There are many film versions that go creative on the matter, some aren't even deformed at all, but all those versions have very little to do with the original story anyway.

According to the movie and musical, the Phantom was born with a disfigured face. We learn this in the song "Down Once More", when the Phantom speaks of his "face that earned a mother's fear and loathing...a mask [his] first unfeeling scrap of clothing." He was put in a carnival sideshow as a very small child.
