He has changed the world mainly because of his work with Black Sabbath, because they were the pioneers in Heavy Metal, and influenced many bands. Actually, many of these bands wouldn't even be there without Sabbath!

It's hard to say. All his initial fame, for a few decades up to the start of the Osbournes, was built on his excellent work as vocalist with pioneering heavy metal band Black Sabbath and his own solo work. More recently, he is (unfortunately) mainly known as the incoherent weirdo from the Osbournes.
