On August 8, 1992, during the Guns N' Roses-Metallica Stadium Tour, he accidentally stepped into the path of one of the chemical flames that was supposed to shoot from the lip of the stage during the song Fade to Black. The fire engulfed his entire left side, putting him in the hospital and rendering him unable to play.

Montreal, Canada in 1992. He was burned after a misunderstanding about one of the pyrotechnic displays.
