Because he like to yell and sceam. Tourette's is a type of disease that causes people to unvolentarily scream, not that Kurt had it, but that's where the title originated from. I don't think Kurt had a particuliar motive to write the song as Kurt was more focused on Nirvana's sound rather than lyrics. Even though many of Nirvana's song sound like they have deep meaning, many of them don't. I think Kurt just had a gift, to him words just happened. I think it's just a fun song to scream your head off to. Nirvana doesn't have anyother song with straight screaming through the whole song.
