Elvis had many ups and downs, as many of us do, concerning his weight.
Also, when he watched his own movies he often complained and commented about himself as "that fat (so and so)".
Hence, Elvis viewed his weight unfavorably and though he took diet pills during this movie he was unhappy with the way he looked thus he wore a jacket.
