Matter accreting into the hole may well reach light velocity before crossing the "no escape" zone, be converted to energy and flash away. While this matter is lost, the flow of energy may repel other matter at a more distant point so that does not continue to fall in. There is some supposition that some stars and some galaxies may have black holes at their cores that are in balance with the external matter shell.
