It usually happens when something organic (either plant or animal) dies. The 'corpse' gets covered in silt at the bottom of a river - and the organic part decomposes - leaving the 'skeleton' of the organism behind. Sediment continues to build up - and the pressure 'embeds' the imprint of the item in its surroundings. Over time - all that's left is a hard rock - with the image inside it.
