As a nurse, this is something I learned to be able to teach at times to women, who, due to a medical or surgical condition, are unable to sit. It is possible, and also a convenient and more healthy alternative when faced with very unsanitary restrooms or when traveling in other countries where the typical Western commode is not available and not used in the culture. Similarly, if you are faced with an immediate need to go and the only usable restroom is equipped solely with a male urinal, after practicing, they can be used as well with this method.
The following steps use terms of human female anatomy, as taught by nurses to female patients, stop reading now (or get over it) if you can't read about body parts without being offended.
It does take a little practice so learning in the shower is the best place to start. But it doesn't take long at all to become capable of using this alternate method.
* If you are where there is a commode but it is unusable due to filthy conditions, make the target as big as possible until you get practiced at this by lifting the toilet lid and seat. Use tissue to lift them, so you don't have to touch the unsanitary commode before following the steps below.
* Stand facing the toilet (or urinal) and lift your skirt or pull down your slacks (you may need to step out of one of the pants legs to be able to do this best with a wider stance until you are more practiced. Tuck the removed pants leg inside the other leg to keep it from touching the floor. Alternately, just remove the pants and hang on a hook if one is available and clean.
* If you are wearing a skirt that could get in the way, tuck the bottom of it inside the waist of the skirt as much toward the back and out of the way as possible.
* With practice you will be able to do this without removing your underwear by pulling the leg opening to the side, but at first it will probably be best to pull them down as described above for the slacks. (Some women even become accomplished enough eventually that they can do this by just opening the fly of the pants, and pulling the underwear to the side.)
* Start with your feet spread to the width of your shoulders or wider if necessary. With practice you will find the best standing position for your body size and shape. You may try bending your knees slightly or not. You can push your pelvis forward and lean backwards slightly if that works better for you. Experiment.
* Use your fingers to gently open your labia. Spreading open both the outer labia and inner labia. If your hands are small, you may need to do this using your thumb and little finger while using your index and middle fingers to pull upwards and outwards to expose the urinary opening just below the hood of the clitoris. Your practicing will let you know how your fingers can be used to adjust the aim and height of the arch of the urine flow.
* To make the urine come out in a stream away from the body and not run down your leg, you need to start the stream with a strong push initially and then once the urine flow is aimed and arcing properly you can relax a little but maintain some pressure to keep the stream going. Then at the end, push again to end with a strong stream instead of a dribble.
* Again, with practice, you will learn how to continue to push enough to maintain the stream and how to hold and keep the labia pulled open to avoid spraying instead of creating a stream.
* Use toilet paper to wipe as usual, if available (when traveling it is always wise to tuck a tissue in your pocket before going into the restroom, since the dirty restrooms often do not have toilet tissue replaced either.)
* Replace your clothing, if wearing a skirt, make sure it is not still tucked up in the waist band before you leave the restroom.
* Do unto others as you would have men do unto you, and put the seat down when finished, using tissue.
* Use toilet tissue to flush the toilet and open the stall door. Thoroughly wash hands unless the sink is also too dirty and then use paper towel to open the door to leave and use the hand sanitizer you carry with you at all times to clean your hands before touching anything else.
