Black holes are the end result of a supernova. Eventually the gravity of the dying star causes it to collapse into itself, resulting in a black hole.
Get enough mass close together and it will shrink into a black hole (about 10 solar masses will probably do nicely but 10000 solar masses will undoubtedly make a black hole if it is concentrated in a small enough volume).
Black holes are created when a giant star collapses in on its own core to be consumed in its gravity, caused by the growing level of density.
