When there is a solution on either side of a barrier (membrane for example) withing a cell, the concentrations of said solution will be different on either side of the barrier. Concentrations do not like to be different and will move from high to low until they are equal.

For example, there are sodium ions on either side of a cell membrane. the concentration is higher inside, so to make the concentrations equal, some salt ions move through transmembrane proteins from the inside of the cell to the outside.

it's called passive because it doesn't use any energy
