Large areas would be scorched by pyroclastic flows and an even larger area would be covered in falling ash. Cars and planes would be rendered inoperational. Roofs would collapse from the weight of the ashfall. People and animals would suffocate. Toxins in the ash would kill plants. Sulfur emissions would lower temperatures worldwide, causing crop failures and leading to mass famine. We may see another "Year Without a Summer" or even several in a row. It would be the single worst catastrophe in human history.
