This first of all requires a lot of muscle activity, which heats the body up more than for someone who isn't having trouble breathing. Second of all, it causes a lot more moisture to be lost through the respiratory system. Dehydration is easier in this situation.
Also, a lot of the people who are having these problems are overweight, and therefore may have a lot more insulating fat and less capable sweat system for their body weight than those who are in good shape, also their percentage of water weight to body weight is less than in a healthy person, so it's much easier for them to form a systemic water imbalance.
