Copernicus' book, De revolutionibus orbium coelestium(On the Revolutions of the Heavenly Spheres), was written in Latin and was extremely technical, so attracted vey little interest at first, with the print run of only 400 copies failing to sell out. It gradually attracted attention and criticism, until the Catholic Church banned the book on 5th March 1616. The ban was lifted in 1758.
In 1633 the Roman Catholic Church forced Galileo to stand trial for his belief that the Earth moves around the Sun. Galileo had to publicly denounce the idea and was sentenced to life imprisonment. He spent the the rest of his life under house arrest. Galileo became blind before he died at Arcetri, near Florence, on January 8, 1642.
Galileo's impact on the universe as we know it was huge. The fact that the earth revolved around the sun was not known at the time of Galileo, and although Copernicus had created a heliocentric model of the universe before Galileo did, Galileo was the first to actually use evidence. By using the first functional teloscope, he proved that the Earth does actually move, and that the sun, not the Earth was at the center of the universe. Although his books were banned by the Roman Catholic Church and the Pope, and his ideas were shown as ludicrous, Galileo is still known as the "father of modern science." He had many contributions in the fields of astronomy, by creating the teloscope, in the fields of mathematics and physics and philosophy. He disputed and disproved the ideas of Aristotle, which were held as true during the time. His revolutionary ideas changed the world we know today.

There isn't a complete, singular list of banned Catholic books as the list has evolved over time. However, historically banned books include works by authors such as Galileo, Descartes, Rousseau, and Voltaire. The Catholic Church's Index Librorum Prohibitorum, a list of prohibited books, was abolished in 1966.
Yes, and not just during the middle ages. The Church maintained an Index Librorum Prohibitorum - a list of banned books - which the faithful were forbidden to read. The Index was abolished in 1966.

No awards. People didn't win awards in this time in history and the Catholic church banned him so he was not popular. His books were burned.
