You get authorization. You didn't say what sort of authorized story you want to write -- it's different for something like a newspaper or magazine than it would be for a book. You always have to contact the person in charge and get permission though -- that's what the word "authorized" means.
This question isn't very clear -- what sort of name are you looking for that might have a number? Story titles come fromwhat you've written, not the other way around. Write your story first, then the title will come to you.
