If abuse or unsafe practices have not been reported, then notifying a manager or supervisor is recommended. Then, contacting local departments in charge of the field would be the next step. Further reporting may be required and is at one's discretion when reporting.

If nothing is done to ensure that it is corrected it is important that you have made a written complaint as if it should go to court or the organisation is investigated, if it's not written down it didn't happen. If it has been formally written and still nothing is done, A Social CAre Worker should take it higher, depending on the unsafe practice. If it is to do with manual handling, medication etc; then it should be reported straight away to sfeguarding.
