Focus charting is a type of nurse's charting used by nursing students. It is composed of three or four component (DAR/DART), depending on your School of Nursing's policy. It is used to focus in on one client problem at a time.
FOCUS CHART: *military time* *date*
Data: the objective data that led you to perform an action
Action: independent/dependent nursing actions you did to resolve/relieve the one problem identified in the data section
Response: subjective (patient reported) data (you want to put this in quotation marks), objective measurement's showing response/s to intervention/s... stay away from imprecise words such as "seemed" or "appeared"
Teaching: (may or may not be included, depending on school policy) what you did to educate the client about his/her problem and/or interventions
