"¿Qué está mal? " is the direct translation, but is very rarely actually used in conversational Spanish.

Many people will often say something like "¿Qué tienes?" (What's your problem?) or "¿Qué pasó?" (What happened) in the same way we in English would ask "What's wrong?"
