It all depends on who your are talking to. If you are speaking informally (ie to a son, daughter, employee, close friend, or someone younger than you), you would use the "tu" form of the verb "haber". Otherwise, if you are speaking informally (like to a boss, parent, other elder, or stranger), you would use the "usted" form.

"Has terminado haciendo ejercicio?" Translates to "Have you finished exercising?" (informally).

"Ha terminado haciendo ejercicio?" means the same thing, but formally.
