If you are using barb less hooks then cover the cat with a soft towel, control the legs, cover the cats head and use pliers to remove the hook in the direction it went in.

With a regular barbed hook it would be best to have a vet remove the hook. Home removal involves pushing the hook through the tissue, clipping the barb and then pulling it out. Not something that should be attempted by an untrained person without access to topical analgesics.
