Ozark is the in house label of Walmart. Your tent was probably made by a company called NorthPole Limited. Provide them with the model number and size and see what they can do; although you might save money buying a new tent complete. You might be able to get it repaired by a sailmaker/canvas repairer instead of replacing the 'top', if you still have it.

If you have the junior dome tent, it's pretty easy. Mine came with two poles, and is 6'x5'. Simply lay the tent out flat, stake the corners, and lay the 2 poles diagonally across it. Feed the poles through the sleeves, and gently push them into the corner pockets one at a time to make the dome. Attach the rain fly on top, and viola. Done.

Red poles make up the right and left sides. Slide the yellow poles through the center pole sleeve. Slide the red roof poles through the right and left pole loops of the tent.3) Raise the tent. Assemble the yellow upright poles and attach them to the yellow roof poles. Raise the center of the tent to its approximate height, by using the quick clamps on the bottom of the yellow center uprights. Assemble the red left and right support poles, attach the support poles to the roof poles. Place the side support poles on the ground near the side stake loops. You can re-order them from the factory at 800-775-1965. Tell them your tent model number and the parts you need. I believe the tent pole part numbers are CPOL-392BL and TPOL-428RD but they will be able to verify.
