Just keep reminding your brother to take his medication. If that gets old then finally let him in on the big secret. That secret would be that you are the only child and your brother is an imaginary brother you invented because you always wanted a brother. Make clear that your parents and other family members only go along with this imaginary brother you created because the Doctor reccomended it was best to do so.
