In a manner of speaking, yes, he did leave his wife and children in Stratford when he went off to London to pursue a career in theatre. However, he presumably did so with his wife's consent and support. He was able to contribute to the support of his family, becoming rather well-to-do from his theatrical enterprises, and returned to them when he retired. He stayed in Stratford until his death.

We have no accounts of Shakespeare's childhood, but we have no reason to believe that his life was in any way unusual for a boy of his class either.

Yes, for about 25 years he lived and worked in London while his family lived in Stratford-upon-Avon. Presumably he got to visit them sometimes, but we are not sure how often.
