Someone called her on the telephone and she picked up the iron. Unfortunately, offensive jokes about people with disabilities still exist. This one doesn't make any sense. Ms. Keller was profoundly deaf and would not have been able to hear the telephone ring that supposedly caused her reaction. She would not have answered a telephone at all. She would not have had a heated iron nearby unless she was doing some ironing, an unlikely activity for her.
