That depends on whether your teacher / instructor has given you a formula to use for it or not. You don't actually need a template to write a poem, but this happens to be one that a lot of teachers request to be written in a certain way. Here is one that many teachers use, based on a poem by George Ella Lyon:

  • I am from _______ (ordinary item), from _______ and _______ (products you might use).
  • I am from the _______ (description of your home, using adjectives and specific details).
  • I am from the _______ (plant, flower, natural item), the _______ (plant, tree, detail of why you remember it or will miss it)
  • I am from _______ (family tradition) and _______ (family trait), from _______ (name of family member) and _______ (another family name) and _______ (family name).
  • I am from the _______ (family habit) and _______ (family habit).
  • From _______ (something you were told as a child) and _______ (a song/saying you learned as a child).
  • I am from (representation of religion, or tradition. Further description).
  • I'm from _______ (place of birth and family ancestry), _______ (two food items representing your family).
  • From the _______ (family story about a specific person), the _______ (another detail), and the _______ (another detail about another family member).
  • I am from _______ (description of family heirlooms, their location, and more description about why they are of worth).
