We do not know for sure whether Shakespeare was faithful to Anne. Certainly she was happy to have him return home to Stratford in 1613--no objections were ever raised. But it is entirely possible that Shakespeare may have succumbed to The Temptations of the city and his lonely exile from his home and family. Thomas Betterton claimed to be the illegitimate son of Shakespeare by his mother who lived on the road between London and Stratford. He might have been lying to boost his image, but we cannot prove it.

If the sonnets reflect personal experience, he had an extra-marital affair with a dark haired, dark eyed woman who has become known as the Dark Lady of the Sonnets. Some scholars speculate that she was a Jewish musician from Venice whose married name was Lanier.
