I believe it means that she wears her life on her face and that's the only "make up" she wears. Great poem. The language is so economical. It's profound, but also takes a light attitude towards the self. Hope I haven't ruined it for anyone. Also hope I'm "right," but, ultimately, a poem is in the mind of the reader.
