The Nurse's remarks lead Juliet to believe that Romeo is dead. She later perceives that it is Tybalt who has died, and at Romeo's hand.
In Romeo and Juliet, Tybalt kills Romeo's friend Mercutio, so Romeo kills Tybalt. Romeo kills Paris and then himself, thinking that Juliet is dead. Juliet wakes up from her sleep and, seeing Romeo dead, kills herself. At the end we find out that Romeo's mother dies because of grief over the banishment of her son.

Tybalt was asking for a fight with Romeo. Since he was married to Juliet and Tybalt was now his cousin, he refused to fight him. Then Mercutio asked to fight Tybalt and in a way kind of tempted him to fighting. Tybalt ended up killing Mercutio because Romeo got in the way when trying to stop the fight between the two. So now Romeo is angry that his friend Mercutio is dead and Tybalt doesn't have a scratch. So, Tybalt and Romeo fight and Romeo kills Tybalt.

romeo marrys Juliet, he kills Tybalt and then gets banished, then he kills himself because he thinks Juliet is dead.
Juliet misunderstands why the Nurse is distraught—she thinks it's because Tybalt is dead but doesn't realize it's actually because Romeo has been banished for killing Tybalt.
