The question is incomplete. The difference between that reality and what?
The main difference between a realist and an anti-realist perspective is their beliefs about the existence of an objective reality. I'm assuming you mean " what's the difference between reality "out there" and "in here". Not much. First, reality is real because you can't dismiss it without acknowledging it. Secondly, the only main difference would be the effect our sensory organs have on reality. But, if you believe in math and science, then, your sensory organs must obey the same laws everything else does, which means that when you feel something or see something, there is only a narrow range of parameters that you can percive those sensations. For example, you can rest assure that the color blue that you see is the same, or almost the same, as the color blue everyone else sees. Your eyes wont see ultra violet, because the construction of your eyes only allow for visible light to make them work. Your other organs work the same way. Another way to look at is, you are basically an organic machine. Machines only work in ways they were designed (usually). It would be like trying to run a gas engine off of diesle. It wouldn't work for long.
