A pressure relief valve got stuck open, but the operator console read that it was closed. This resulted in many operator errors and the eventual exposure and meltdown of the upper third or so of the core.

A stuck open valve and a design error on the control panel showing the commanded position of the valve not the actual position. This kept the operators confused long enough for a severe LOCA to occur (through that valve) to allow partial meltdown of the core.

Three Mile Island IS the site of a Nuclear Reactor complex.

There was a serious accident relating in the release of Radio Active gasses and water.

There were no deaths and even though the radioactive material release was large no health effects have been measured.

The real reason this was so controversial that even the Government didn't know how bad an accident it was until a full investigation was ordered by the president.
