A pressure relief valve got stuck open, but the operator console read that it was closed. This resulted in many operator errors and the eventual exposure and meltdown of the upper third or so of the core.
A stuck open valve and a design error on the control panel showing the commanded position of the valve not the actual position. This kept the operators confused long enough for a severe LOCA to occur (through that valve) to allow partial meltdown of the core.
