I think they start with some meaning, and it is a matter of expressing the meaning through their craft. It is a fluid kind of process, even for a poet who works, re-works, re-writes and struggles hard doing it. I don't think poets will sit down and say, for example, "The meaning I want to convey I have to convey with iambic pentameter", or "I must avoid alliteration here because that conveys the wrong meaning". Poets start with a meaning or a concept, and then they explore language and the sounds of language. The meaning is there, and the poet sets out to write or to orchestrate it with the sounds of her/his language. The pulse, or prosody, of the piece will be important, but in what way it is important may be a surprise to the poet as the writing progresses. For example a poet may be creating a sound/meaning and decide to use the form of heroic verse either to lift the meaning intended, or perhaps to mock it. It is the whole body of the work that will provide context for any useful interpretation. And frankly, for the most brilliant poets, I think the meaning often connects with the proper prosody without a lot of conscious planning; the musical and prosodic grasp of language is so deep that it comes out like the most brilliant instrumental improvisation.
