One failure of IO's was by the UN for its choice of non-intervention in the Rwandan genocide. According to UN rules, it was only allowed to intervene with Rwanda's consent, and also with there being a cease-fire in place. Also, the UN Secretariat assessed the genocide as a result of the ensuing civil war (outside UN intervention), and not a case of ethnic cleansing and crimes against humanity, which would have given it the authority to intervene. Intervention would have likely saved a significant number of lives but would have also put the the UN's reputation at stake. 800,000 Rwandans were killed in 100 days.
