Pretty much the same way religion started. Someone smarter than the average claimed they had something special that made them unique and stupid people followed him. Fast forward a few hundred years and you see people are still following the same thing without even knowing why! Idiocracy is and has been in human's DNA.

Well, back in the Egyptian days, everyone was equal. That was, until, they decided they wanted to have a leader. So they appointed a pharaoh, which is basically a "king." After ancient Egypt died out, people still liked the idea of a leader, but decided to call it a different name. So they came up with "kings, queens, princes, and princesses". They also believed that from then on, anyone who descended from the leader would be in line to inherit the throne after the king or queen had died.

If you're talking about the British monarchy, then that's a bit different. The Royal family started after the Viking raids in the ninth century which made the Anglo saxon kingdom of Wessex emerge as the dominant English kingdom. Alfred the Great managed to secure Wessex and Mercia taking the title of "King of England". This was the start of the British monarchy.
