A persons identity and self esteem can be linked with their spiritual well being by actively participating in the church. The three are not normally connected, but the connection with the church for someone who is involved can enhance self esteem and identity.

For purpose of clarification, let me define each of the 3 subjects in your question:

An individual's identity is composed of beliefs, qualities, culture, upbringing, biological make up and other factors that make a particular person or group different from others. When we speak of identity, we speak of a person's uniqueness.

On the other hand, Self-Esteem is one's overall sense of self-worth of personal value.

One's definition of spiritual well-being depends on how he view spirituality. If an individual was raised from a conservative Catholic family, part of his identity is his belief in the Holy Sacrifice of the Mass and other symbolic acts within the practice of his religion.

Self-esteem may or may not be affected in such a way that self-esteem develops from interacting with others.

An individual's self-esteem can be develop from joining religious organizations. For instance, as mentioned above, the individual may have joined the choir. The interaction with others as well as the overcoming one's shyness paves the way for development of self-esteem.

With continuous exposure to other people, the individual's level of self-esteem rise until he reaches old age.

It is a person's identity that can be influenced by his spiritual well-being.

If an individual feels more self-worth by developing his spiritual well-being, then it does affect his self-esteem. It is a case-to-case basis.
