I recommend an older work on this topic; "The Crowd" by Gustave le Bon. He, like others since, postulate that a mob forms around a person or a singular event (in much the same way as a particle in a super-cold solution can make it harden instantly) which causes the formation of the mob. The mob follows it's own rules (like all chaotic 'systems') and they are almost never the rules of the individuals making up the group.
Mobs can form through a variety of mechanisms, such as social influence, emotional contagion, or a shared sense of identity or grievance. Factors like anonymity, conformity, and deindividuation can also play a role in mob formation, leading individuals to act in ways they might not typically behave when part of a larger group.

Passive mobs need light to spawn, and they only spawn on grass. If the aggressive mobs are spawning, then it must be night and the passive mobs will only spawn near torches, pumpkins, glowstone, etc. However, Notch MAY have fixed this in 1.6, but I'm not sure.
Mobs are enemies, so I suppose that there wouldn't have been.
No, mobs will not spawn in water or ontop of half slabs.

In Minecraft Classic, mobs can't spawn, so they can't exist naturally. If you play the Survival Test, and when mobs spawn there, you can save it and open it in creative mode so then the mobs can stay there. However, in Minecraft Creative Mode, mobs can spawn, but they can't hurt you because you have no healthbar.
