The only thing anyone goes to hell for is because they rejected Jesus Christ as Savior and God. Disobedience to God is why people go to hell. So the only way a piercing will cause someone to go to hell is if God tells them not to get it and they do it in defiance. It would be the life of defiance, and not that single act, though.
