If you are a baptized Witness you can ask one of your elders in your congregation and they will assist you on how to get your baptismal record. Normally the branch office in your country will release it for you provided you tell them the date of your baptism and other pertinent informations.
If you are catholic, you will need a record to receive first communion, confirmation, and marriage sacraments. The church where you were baptized should have it, but if no longer there, the receiving church should have it. If not, you will need an affidavit from a witness (parent, godparent) who can attest to the baptism. If no witness, you may need a conditional baptism. If you are not catholic, you will need a record to be married in the catholic church to a catholic party. Int his case, if no record is available, or no witness, a declaration of nullity form is used where baptism is in question; this can be approved by the bishop if no other impediments exist.
You could contact your former congregation and make a request but since only active publishers (preachers) are counted as Jehovah's Witness members, its possible they have been disposed of. There are no "baptismal records" as such, what is held is a record of you last monthy activity in the preaching work as well as your last know contact details, date of Baptism and (depending on the country and the legal system) inforation relevant to the safety of their members such as whether you are a convicted child abuser. You don't need one if you are former Jehova's witness, you should know when you were baptised & if you are no longer congregating then don't even worry about it you made a personal decision & that's it.

It depends. For example, there are numerous Witnesses who were put in prison for not serving in the military. They have a record as fellons; and have never repudiated what put them in prison; no sorrow at all. They remain full fledged Witnesses.

From a scriptural standpoint, there is no reason for a baptism to be private.It CAN be, if the person wishing to be baptized is highly self-conscious or (in extreme cases) fearful of retribution, but in the New Testament record, baptism involved hundreds, and even thousands of people at a time (Acts 2:41).
