Hail Mary, full of grace, the Lord is with Thee. Blessed art thou amongst women, and blessed is the fruit of thy womb, Jesus. Holy Mary, Mother of God, pray for us sinners, now, and at the hour of our death. Amen.

The first part of the prayer is a direct quote from the Gospel of Saint Luke, up to the word, Jesus, which was added later. The second part was added later, in the second millennium I believe (after the year 1000 AD).
