The baby is given wine as anesthesia and is held by either the father, a close male relative, or a respected rabbi. A device, called a shield is put in place to make sure that only the foreskin is removed then the skin is quickly removed.

As a side note, in a Jewish circumcision, less of the foreskin is removed than what is removed in modern hospital circumcisions.
