According to Jewish tradition, it doesn't mean a different covenant, since two verses later the prophet continues "I will place my Torah among them and on their hearts."

A Jewish person would consider it offensive because it could insinuate that that the Torah is no longer valid, having been replaced by something new (New Testament). Usually, Jews do not find the term Old Testament offensive, just inapt. The Old Testament is different from the Jewish Bible or Tanakh in that it has additional books, a different order to the books, different translations of key words (usually in support of Christology) and additional material in certain books. As a result, the Jews prefer to say that they read the Tanakh than that they read the Old Testament.
There are 39 books in the Old Testament and 27 books in the New Testament. The word Testament means covenant or promise. The Bible constantly tells us in one form or another that God is with us. A covenant (promise). The New Testament is not of sacrifice but of Love. When Jesus died on the cross the new covenant began. We still must obey all of the commandments in the Old Testament but the new covenant began. All through the new Testament it tells of how God is with us. He knows we are not perfect and He does not expect us to be. But if we ask for forgiveness, at anytime, God will forgive us. He is always everywhere and always with us. If we ask for forgiveness and really mean it, God must and will forgive us. Instantly and forever. Never to remember those shortcomings again. We can do this because God is with us always. He promises.

There are many verses in the Bible saying that Jesus fulfils the new covenant, and here is one: And for this cause he is the mediator of the new testament, that by means of death, for the redemption of the transgressions that were under the first testament, they which are called might receive the promise of eternal inheritance (Hebrews 9.15)

The New Covenant (or New Testament) is the promise that God makes with humanity that He will forgive sin and restore fellowship with those whose hearts are turned toward Him. Jesus Christ is the mediator of the New Covenant, and His death on the cross is the basis of the promise. The Bible doesn't say that God will send anything except the Holy Spirit to those who believe. Under the new covenant life is not about obeying God's commandments but being led by God's Spirit. However God's Spirit will led a person to obey God's commandments. If Christians obey God they will show all people that they are Jesus disciples.
