The name "Presbyterian" is based on a Greek word, presbyter, meaning "elder." Presbyterian congregations elect "ruling elders" from among themselves to serve on a board (or "court") often called the "Session." Elders formerly were elected for life and were only male; now, in some varieties of Presbyterian denominations (there are many), elders are elected for terms, and may be female. The Session is the governing body of a local congregation, presided over by the minister.

Representatives are periodically (usually annually) sent from the local Session to "Presbytery," which is responsible for ordaining ministers and approving their "call" to constituent congregations. Presbytery has general oversight over its congregations, and is presided over by an annually elected "moderator." Presbyteries send representatives to higher "courts" called "Synods" and often "General Assemblies, which are essentially organization of presbyteries." From these higher levels, recommendations for changes which would affect the entire denomination are formulated and sent to presbytery level for approval.
