The Vatican City does not have an office called "City Manager."

The Pope is the head of state for the Vatican but manages this independent city state of about 800+ residents through a Governorate. The Governorate is the group of Departments through which the President, assisted by a Secretary General and a Deputy Secretary General, exercises executive power.

It is divided into numerous Administrations and Central Offices, common to many cities around the world.
