It is simply used as an official record of your baptism. The only importance it has is because without that you can't prove you were baptised, if yours is damaged you can easily get a new one though. So you know the certificate is noting but a sheet of paper. It holds no religious value at all.

You need a certified birth certificate, not a baptismal certificate. It is only important if you have no birth certificate and a letter from the state saying that there is no record after a detailed search of your birth certificate.

The front of the baptismal certificate provides a place for the name and address of the church where the baptism took place, then it specifies the date, the parents, the name of the sponsors, a place for the current priest to sign and date, and it should have the seal of the Church where it was issued. The back of the baptismal certificate has places to record the date and place other sacraments were received and the date and place where first Holy Communion was received.
