It's like a hub cap. They are smaller though and only cover the bolts.
Simple... You remove the wheel from the car and punch out the center cap from the backside of the wheel.

Remove the center cap, washers and C-clip and the wheel should slide off.

It is extremely easy and simple to replace your center caps on the Nissan Xterra... --- the center caps are held in place by a series of spring clips... --- using a long screw driver and a rag (to protect your wheel), gently pry the center cap off...by moving around the circumference of the center cap with a prying motion. --- reinstall the center cap by "snapping" it back in place...double check to make sure you have snapped it back in place securely.
If you are asking about the center "Hub-Cap" on the stock wheels, and of their removal, you have to remove the wheel from the car first. On the inside of the wheel, there are I think three 5/16" head screws that screw through the wheel into the plastic hub cap. Once you remove these screws, the cap is free from securement and will simply fall off.
The only way to remove it without damaging it is to remove the wheel and tap cap off from the inside.
The centre cap has the same function as most hubcaps. It is mainly for aesthetics and to protect parts of the wheel from dust, debris and water (to a point).
take it to your dealer or service center and tell them to change the steering wheel that's what i did or if you tried that there should be a cap on the front of the wheel and you snap it open and there should be bolts.
