Currently there are no technologies that I am aware of for quadriplegics to drive cars that are certified for use. There are technologies for moving a motorized wheelchair with a mouth "puffer" or "joystick", however typically quadriplegics are not independent enough to be able to place themselves into a wheelchair after driving is completed. This means they must rely on the help of another for driving purposes as there's no technology present that can be used once a quadriplegic person arrives at the destination for the transfer of themselves to the wheelchair.

Paraplegics do have systems that assist for driving purposes. It's a bar of buttons that typically sit below the steering wheel that they can control with their fingers for braking and acceleration. These systems do come at a cost and are aftermarket systems, so some paraplegics opt for "alternative devices" such as canes attached to objects to push the pedals that are still located on the floor.

As a side note the buttons used for those with paraplegia are also useful for those that are vertically challenged (aka short statured people that literally cannot reach the gas/brake pedals of the car).

As far as I know, only one doctor needs to certify a patient for hospice, and it can be the patients doctor that does this. I've work with hospice for 20 years, and in that 20 years I've never heard that it took two doctors or just the medical director with hospice to certify a patient to be enrolled in the hospice care program. The patient does hwoever need to be deemed terminal for the hospice care program to be considered.
