Click on Messages on the top of the page.
You click on a person's profile, and you will see a button on the page with + Follow. Click on that.

No there's no way of doing this.
You cannot follow Twitter user directly from Facebook. If you see that some one has integrated his twitter account with Facebook and his tweet is poping up in your news feed (Facebook), you will see a twitter symbol below his/her post. Click on it and you will redirected to Twitter page. If you are log in on Twitter you can follow the person.

Nobody else will see it in their stream (although it will show up on your profile page and in Twitter search). If you tag a username anywhere but at the start of the tweet, everybody following you will see that message.
